If you've ever had a cold sore, you've had herpes — specifically HSV-1. It's one of the most common viruses in the world. HSV-2 is the type more commonly associated with genital herpes. Both belong to the same family of viruses, both are lifelong infections, and both can be managed with the right treatment.
So what actually sets them apart? Here's everything you need to know — from how each type spreads to how they're diagnosed and treated.

What Is HSV-1?
HSV-1 is the herpes simplex virus type 1. It's extremely common with about 50 to 80 percent of American adults carrying it.[1] Most people pick it up in childhood through non-sexual contact, like a kiss from a parent or sharing a drink with a friend.
HSV-1 most often causes cold sores: small, fluid-filled blisters that appear on or around the lips. But it doesn't stay limited to the mouth. HSV-1 now causes roughly half of all new genital herpes infections in the US — largely through oral sex.[1]
What Is HSV-2?
HSV-2 is herpes simplex virus type 2. It's primarily spread through sexual contact and most commonly causes genital herpes. It is estimated that genital herpes affects 1 in 8 people in the US between the ages of 14 and 49.[1]
HSV-2 tends to cause more frequent recurrences than HSV-1, especially in the first year after infection. It can also cause oral herpes, though that's much less common.[1]
How Each Type Spreads
Both types of HSV spread through direct skin-to-skin contact with an infected area. The biggest complication: the virus can spread even when no sores are visible. This is called asymptomatic shedding, and it's one of the main reasons herpes is so widespread.
How HSV-1 Spreads
- Kissing someone with an active cold sore — or even without visible sores
- Sharing utensils, drinks, lip balm, or razors
- Oral sex (this is how HSV-1 spreads to the genitals)
- Touching an infected sore and then touching your own mouth or genitals
- High-contact sports like wrestling or rugby
How HSV-2 Spreads
- Vaginal, anal, or oral sex with someone who has HSV-2
- Direct skin contact with an infected area — even without an active outbreak
- From a mother to her baby during childbirth, if the virus is active at that time[1]
Neonatal herpes (transmission during birth) is rare, but it can be serious. Pregnant people with herpes should discuss their status with their provider so precautions can be taken.[1]
What are the Symptoms of Each Type?
Both types of herpes can cause outbreaks that look similar — fluid-filled blisters that break open, crust over, and heal within one to two weeks. The main difference is location.
Symptoms of HSV-1 (Oral Herpes)
- Cold sores or blisters on or around the lips, mouth, or gums
- A tingling or burning feeling before the sores appear
- Sore throat or swollen glands (especially during the first outbreak)
- Fever, fatigue, or muscle aches with the initial infection
Symptoms typically appear 1 to 3 weeks after exposure and resolve within about 3 weeks.[2]
Symptoms of HSV-2 (Genital Herpes)
- Blisters or sores on the genitals, buttocks, or thighs
- Tingling, itching, or burning before sores appear
- Pain or burning when urinating
- Swollen lymph nodes in the groin
- Fever, body aches, and fatigue during the first outbreak
Symptoms usually appear 2 to 14 days after exposure. The first outbreak is almost always the worst one. After that, recurrences tend to be shorter and less severe.[3]
When There Are No Symptoms at All
Many people with herpes — both HSV-1 and HSV-2 — never have a noticeable outbreak. They may carry the virus for years without knowing it. This is one reason regular STI testing matters, even when you feel fine.[1]

After the first infection, HSV lives in nerve cells near the site of infection. It can stay dormant for months or years, then reactivate when the immune system is stressed. The virus travels back to the skin, where it may or may not cause visible sores — but it can still be transmitted either way.
How Each Type is Diagnosed
You can't tell which type of HSV you have based on symptoms alone. Testing is the only way to know for certain. There are two main types of tests:[1]
Swab Test
- Requires an active sore or blister
- A healthcare provider swabs fluid from the sore
- Can be run as a viral culture (takes several days) or a PCR test (faster, more sensitive)
- Can distinguish between HSV-1 and HSV-2
Blood Test
- Looks for HSV antibodies — proteins your immune system makes after infection
- Can be done with or without an active outbreak
- Can distinguish HSV-1 antibodies from HSV-2 antibodies
- May not be accurate until 12–16 weeks after exposure (it takes time for antibodies to develop)[4]
One important note: because HSV-2 increases the risk of HIV infection, it's worth asking your provider about HIV testing at the same time if you're being tested for genital herpes.[1]
Treatment Options
There is no cure for herpes. But antiviral medications can significantly reduce outbreak frequency, shorten how long outbreaks last, and lower the chance of passing the virus to a partner.
Common Antiviral Medications

These medications work by blocking the virus's ability to replicate, which limits how much damage it can do and how easily it spreads.[1]
People who get frequent outbreaks (typically defined as 6 or more per year) may benefit from daily suppressive therapy — taking a low dose of antiviral medication every day to keep the virus in check. Talk to a provider about which approach makes sense for you.
In addition to antivirals, these can help ease discomfort during an outbreak:
- Warm baths to soothe genital sores
- Loose-fitting clothing to reduce irritation
- Over-the-counter pain relievers like ibuprofen or acetaminophen
- Topical creams for cold sores (available OTC)
How to Reduce Your Risk
There is currently no approved vaccine for either type of HSV. Prevention focuses on reducing direct contact with the virus.
For HSV-1
- Avoid kissing or sharing items like utensils or lip balm with someone who has an active cold sore
- Avoid oral sex when a partner has an active oral herpes outbreak
- Wash your hands if you've touched a cold sore
For HSV-2
- Use condoms consistently — they reduce (but don't eliminate) the risk of transmission
- Talk openly with partners about STI status before sexual activity
- If your partner has HSV-2, daily antiviral therapy on their end can help lower transmission risk[1]
- Get tested regularly for STIs, even if you feel fine
Complications to Know About
For most people, herpes is a manageable condition. But in some cases — particularly when the immune system is weakened — it can cause more serious problems. According to Johns Hopkins Medicine, these can include:[3]
- Herpes meningitis — inflammation of the membranes surrounding the brain and spinal cord
- Herpes encephalitis — inflammation of the brain itself (rare but serious)
- Herpes keratitis — an eye infection that can threaten vision[5]
- Neonatal herpes — transmission during childbirth, which can be severe in newborns
People with HIV or other immune-compromising conditions are at higher risk for these complications. If you're immunocompromised and have HSV, it's especially important to be under regular medical care.
